| TapTone offers application evaluation as a service to our customers. By testing with actual production samples, we can recommend the appropriate system and the performance capabilities that can be expected.
Our policy is to provide up to four hours of sample testing at no charge. If a more involved study is requested or recommended, a proposal for the additional effort will be generated. The cost of shipping samples to Teledyne Benthos is the responsibility of the party submitting the samples for evaluation, for both domestic and overseas shipments of samples sent directly to Teledyne Benthos, Inc.

Samples Originating Outside the United States:
All samples sent to Teledyne Benthos for sample test/evaluation purposes from outside the United States must have a Prior Notification form filed with the FDA before they will be allowed into the country. Please note that there is a specific form to be used depending on the shipment method selected. Forms for all shipping methods are available from www.fdaregistrar.com. Use the form that is appropriate for your shipping method.
Once you fax the Prior Notification form to the FDA Registration Office you will receive an approval form within 24 hours. This form will have a bar code along the top area and it must be enclosed with all shipping documents. A copy of the form should be sent to Mike Mierjeski at our office so he can assist with the customs process if necessary.
The forms associated with shipment by sea or air require an FDA Registration number by the shipping company and the recipient. Shipping by Express Carrier requires the Recipients number only. The Teledyne Benthos, Inc., (recipient) FDA Registration Number is listed below.
For overseas shipments, documents should reflect that the contents are “SAMPLES FOR EVALUATION ONLY – NO COMMERCIAL VALUE”. Additionally, the shipping documents should be marked “FREE DOMICILE” to indicate that the all freight charges, and any customs or brokerage charges, are to be paid by the consignor.
Please note: To ensure that samples are not detained by Customs, we recommend that the company shipping the samples also be registered with the FDA. Additional information can be found at www.fdaregistrar.com.
